Procedure for Visa Screen (CGFNS)

1. Documents to be posted to ISHA office:
   1. Covering letter requesting for the Visa Screen
   2. Application form (CGFNS)/ or concerned authority – Original with your signature only (unfilled)
   3. Duplicate copy of CGFNS/ concerned authority application (Filled by the applicant)
   4. Transaction copy
   5. Life membership e-certificate (made available in your members page)

4. Every information in the duplication application form should be filled by the applicant ONLY. Incomplete applications will be processed as it is and ISHA will not be responsible for any rejection by CGFNS/Concerned Authority. No further communication will be done with the applicant or CGFNS.
5. IMPORTANT NOTE: All the letters will be posted through post/courier to the concerned authorities or to the applicant based on the request made (to CGFNS office, USA or addressee) in the covering letter.
